Most providers do not charge a set-up fee, just a monthly payment.

NTL (re-branded as Virgin Media) provide what is called ‘Cable Broadband’, requiring you to have an NTL cable into your house. Not everyone can get this service, it depends where you live. If you can get it however, from personal experience the service is excellent, and they are currently doing a deal of £10 per month for the first year (with a £25 set up fee)!
